Senior Contracting Specialist

The Senior Contracting Specialist is responsible for the full range of high-level contracting, provider relations and service interactions with providers that ensure our patients have superior access to the highest quality, cost-effective providers of healthcare services.

SNP Coordinator

Under the supervision of the Director of Medical Management, this position is responsible for coordinating the documentation and required activities for patients enrolled in a Special Needs Plan.   Through enrollment data, communication with brokers and the Health Plan[s], SNP patients will be identified and processes managed to ensure confirmation of SNP eligibility and timely appointments with providers and the Care Team.   Patients will be guided to enhance the achievement of the Quadruple Aim:  improved outcomes, improved experience of care for patients and providers and lower healthcare costs.

IP Coordinator

Under the supervision of the Director of Medical Management, this position is responsible for validating, integrating and coordinating the P3 inpatient list for all local acute and post-acute facilities.    Through electronic interfaces, telephone calls and other sources of admission notification, discharge and transfer, this employee is responsible for an accurate patient census for all P3 admissions.  He / she coordinates with assigned hospitalist for each facility.   Appropriate patient identification enables P3 to  enhance the achievement of the Quadruple Aim:  improved outcomes, improved experience of care for patients and providers and lower healthcare costs.

Care Navigator

Under the supervision of the Director of Medical Management, this position is responsible for assisting the nurses with Medical Management processes that do not require RN intervention, based on the scope of practice within the state.   Through case finding, data and other tools, high risk patients will be identified and guided to enhance the achievement of the Quadruple Aim:  improved outcomes, improved experience of care for patients and providers and lower healthcare costs.

RN Care Manager

This position is responsible for ensuring that assigned patient population(s) receive continuous, comprehensive and coordinated care throughout the continuum.   Through case finding, data and other tools, high risk patients will be identified and guided to enhance the achievement of the Quadruple Aim:  improved outcomes, improved experience of care for patients and providers and lower healthcare costs.

RN – Utilization Management (UM) Nurse

The UM Nurse is responsible for compliance with the Medical Management Program Description of P3 Health Partners regarding Utilization Review, inclusive of prior authorization, on-site and telephonic, concurrent and retrospective review, discharge planning and transitions of care to the appropriate level. The UM Nurse adheres to the standard operating procedures that support achieving the Quadruple Aim:  improved outcomes, improved patient experience, improved clinician experience and decreased cost of care. The UM Nurse will integrate the functions of utilization review, discharge planning and resource management into a singular effort to ensure, based on patient assessment and best practice, that care is provided in the appropriate setting utilizing medically indicated, contracted resources.

RN – Admission and Concurrent Review Care Manager

The Care Coordination Pharmacist serves as the clinical resource for health services initiatives.  Responsibilities will include developing, implementing and monitoring clinical programs for P3 Health Partners members. Care Coordination Pharmacist is also responsible for regular analysis of pharmacy data, and researching the literature to identify quality and cost containment opportunities, in collaboration with vendors, clinical pharmacy staff, Medical Directors, Medical Management Leadership and staff.  The Care Coordination Pharmacist will be actively involved in collaborating with other Health Services teams to develop and implement healthcare initiatives, address ongoing clinical service needs, managing MTM and existing clinical programs, and deliver clinical outreach to members to improve health outcomes.

Pharmacy Technician

The Pharmacy Technician will assist the Clinical Pharmacy Team in assessing pharmacy utilization and implementing medication management programs and strategies for P3. Under pharmacist supervision, the Pharmacy Technician will assist in improving patient outcomes and promoting the safe and effective use of medications to improve quality and the total cost of care.

Must have extensive knowledge of the applicable federal and state rules as well as a solid working knowledge of managed care operations, compliance program structures, and audit methodologies. Must have the highest level of integrity and ethics and demonstrate professional representation of the company.

Coding Specialist

The Coding Specialist supports the providers by ensuring documentation supports the submission of relevant ICD-10, CPT and HCPCS code information in accordance with national coding guidelines and appropriate reimbursement requirements.  S/He ensures member encounter data (services and disease conditions) is being accurately documented and relevant procedural codes and diagnosis codes are captured.
