P3 Senior Wellness Centers give you easy access to local providers who can look at your full health history, including your recent visits to other services and providers, and work with you to build a plan to meet your health and lifestyle goals.
What you can expect when you visit
During your wellness visit at one of our centers, you will receive a one-hour, comprehensive session with one of our nurse practitioners or care providers. They will conduct a wellness exam, including a comprehensive overview of your medical records, listen to your concerns, make note of any recent symptoms, review the medications you have been prescribed, and, if needed, connect you with community resources to provide transportation, food, support group contacts and whatever you need to stay healthy.
We will put together a care plan to help keep you well, or to manage any current conditions you have, all with the goal to keep you healthy and out of the hospital. After the visit, P3 works with our primary care providers — sending them records, coordinating your wellness plan, and making sure you receive the care you deserve.
