What is P3@Home?
P3@Home visits are not intended to replace primary care providers.
Instead, P3@Home providers will work in PARTNERSHIP with primary care providers to give your patients the CARE they need when they need it MOST.
P3@Home providers are AVAILABLE to provide health services in the COMFORT and PRIVACY of patients’ own home who are suffering from chronic conditions or having trouble attending traditional office visits.
P3@Home will also visit your home for wellness and comprehensive exams so we know your numbers when you are feeling your best.
How do I Qualify?
Patients may QUALIFY if they have any of the chronic conditions below:
- Chronic heart failure
- Diabetes
- High blood pressure
- Cancer
- Dementia
Comprehensive Visits
Take charge of your health with an ANNUAL COMPREHENSIVE WELLNESS VISIT. During this 45-minute appointment we will:
- Review all past medical history and schedule screenings and tests to create an effective care plan just for you
- Ensure you get the resources you are entitled to
- Check in with our clinical team about any health issues
- Prevent gaps in your healthcare
- Discover health problems while they are minor and manageable
- Focus on maintaining your wellness, not just treating an illness or chronic condition
All information collected will be sent back to your primary care provider for seamless coordination of care.
Screenings and tests
When we catch diseases and illnesses early you have more treatment options to keep you well. The mobile clinic will offer onsite testing for A1C (blood sugar), retinal eye exam for diabetics and Quantaflo. We will also assist patients in scheduling additional appointments at a convenient site of care for bone density screenings, spirometry and mammograms to promote early detection and prevention.
Vaccinations
Get your flu shot, not the flu. Flu shots will be available in fall of 2023.

Make the most out of your visit by bringing the following items with you:
- Photo ID
- Health insurance card
- All medications you currently take
COVID-19
P3 is committed to protecting patient safety during COVID-19 by following CDC guidelines. We are taking several steps to keep patients safe while in our care. These measures include social distancing limiting capacity in the mobile clinic, wearing masks at all times, hand washing before and after visits and disinfecting all surfaces with EPA-approved cleanser.
