Navigating healthcare can be confusing. Our friendly and knowledgeable P3 Care Team is dedicated to helping you get the care you deserve. Our team may contact you if you:
- Haven’t seen your primary care provider in the last year
- Have recently been discharged from the hospital
- Have been diagnosed with multiple chronic conditions
- Have visited the ER several times
- Have recently transitioned your care

Our care team helps patients get the right care at the right time. Instead of going straight to urgent care or the emergency room, call us first.

Blue Cross and Blue Shield of Nebraska Senior Wellness Centers are designed to support your health, comfort, and independence. At our Senior Wellness Centers, our focus is on preventive care helping you stay well, manage existing conditions, and address concerns early.
If your care team recommends an in person visit, our Senior Wellness Centers provide a convenient place to receive screenings, wellness visits, and personalized support in a setting designed specifically for our members.
